Hallucination and Saving Grace If you've never had a hallucination, it's very hard to realize just what they are. The person who is caught in this state has no way of knowing that the people and places he interacts with are all in his mind. Some people and acts insinuate themselves into the hallucination, and the patient can't distinguish the real from the hallucinatory. That's why it sometimes appears to nurses and family that the patient seems to somehow connect with you, and then suddenly, his mind is off somewhere else. The patient may recognize that you are there, but can't separate his "mental story" from the nurses' reality.
