• Sample of an Application that was Approved - and don't worry - you don't need to be this debilitated to be successful. Just look at the way the applicant documented things.
• Use this Quick Checklist to give you an idea of what you will need to document in order to complete a disability claim application. This page includes a letter for your doctor to use as a sample.
• Before you turn in your application, please read Winning Your Social Security Claim: 15 Mistakes you Cannot Afford to Make. And yes, I know you may think the author it advising you to hire a lawyer because he is one. But my private insurance company doctor admitted to me (reluctantly) that few people win their claims without an attorney.
