Sudden onset stuttering

Our son is 3 and 1/2 and has been communicating and speaking well since he was one year old. Within the last three weeks his speech has been on a sudden decline. He is stuttering very badly and is becoming increasingly frustrated with his sudden lack of speech to the point that he is hitting himself on the head and calling himself "stupid". There have been some changes of recent ie; new sibling, started preschool. How do we know the difference between these potential stressors and what could be a "real problem" with his speech?
