Do you know of an effective process to ensure that people with developmental disabilities can transition successfully from public school to life in the community? If you do, why do you think they are effective. If you don't, what do you think should be tried and why?

I do not know how helpful this information would be for residents in Canada, but here in the US (more specifically New York), young adults (aging out at 21) aging out of Special Education can be a rewarding process for some and others a disaster.

For a rewarding experience many factors come into play.

1. Does the parent or guardian understand the individuals delay and diagnoses? If so, understands the options available?
2. Has the current school prepared the individual for the next step? (Example, a mentally retarded individual functioning between the moderate to mild range, has workshop skills or possible supportive employment been introduced?).
3. Beginning the paperwork for transiting: Has the appropriate applications / referral packets been completed and sent? Was it sent at least 9 – 12 months in advance? Parents should be informed at least 6 months prior to individuals age out of placement elsewhere.

The above is based on individuals with a developmental delay. Depending on the individuals need for additional special services (if any) additional services varies.
