I am looking for information on the use of portfolios in PE. If you have used one what did it look like? Was it skill/age based? Fitness goal oriented? How was the management of the system? I am looking for any help you have.

I have been looking into this for some time now. I just attended a Massachusetts conference on Assessment in PE.
It was very informative.
I decided to keep things simple.
The actual portfolio itself is a "legal size" manilla envelope. Each student will have their name on the tab.
1st grade - color the outside
3rd grade - color the back side.
5th grade - take the portfolio home.
Each grade - gather whatever you use for assessment tools and have the child put it in their folder. At the end of the year staple the grade work together. Add another folder to the packet and the "end of year things" go in there.This leaves the front folder free for the current year.
Assessment tools you use will be based on the focus of each grade level.
For instance we talk about fitness in the primary grades but we don't "test" until 3rd grade. 3, 4, 5 grades will have their fitness tests in it along with gymnastics skill sheets, rules review, etc.
1st grade may keep their bowling scores.
2nd grade will keep their "step & throw" assessment sheet.
Put in whatever has been generated. It should be great for parent conferences.
Store them in the those big plastic crates. Hanging file folders will keep each classroom separate.
It's a start.
