Obsessive Compulsive Disorder Questionnaire

If you have or if you know somebody with Obsessive-Compulsive Disorder, I would greatly appreciate if you would take the time to complete this questionnaire. All information is confidential and it will be used strictly for research purposes. You do not have to put your name or any other personal information if you do not wish too. Thank you very much.Part One- This portion is for those who have a form of Obsessive-Compulsive Disorder. If you do not have OCD then please skip to Part Two. 1) Age 2) Gender 3) How long have you had Obsessive-Compulsive Disorder? 4) Please explain in detail what form of OCD you have and include examples. 5) Do you find that OCD makes it difficult for you to perform daily activities? Explain. 6) How do other people react to your OCD? 7) Do you get frustrated with your OCD? Why or Why Not? 8) Please list any other feelings that you get from having this illness or feel free to make a general comment on Obsessive-Compulsive Disorder. Part Two- This portion is for those who know somebody with Obsessive-Compulsive Disorder. If you do not know anybody with OCD then please do not complete this questionnaire. 1) Age 2) Relationship to person with OCD 3) How long has this person had Obsessive-Compulsive Disorder? 4) Please explain in detail what form of OCD this person has and include examples. 5) Do you find it difficult living or being around this person because of their OCD? Why or why not? 6) Do you and this person argue over things that are in relation to their Obsessive-Compulsive Disorder? Explain. 7) Do you find that their Obsessive-Compulsive Disorder makes it difficult for you to perform daily activities?
