How do I obtain health coverage?

I have been diagnosed with MS for a little over a year. My question to everyone is...How do you obtain health, dental, and vision insurance once you are not working and COBRA has expired? I have called my health insurance company, (GHP) and I was advised that they do have a "conversion" plan available, however, the present monthly rate for it is $500.00 per month. I got discouraged with that information and never called about dental or vision coverage. If I am not working, how am I expected to come up with these absorbent monthly premiums?
