actually had a death as a result of peanut butter allergy. The school knew the child shouldn't eat peanut butter, but on a field trip he consumed a cookie that contained peanut butter from a packed school lunch. The school packed the lunch and the kid had no idea peanut butter was an ingredient. Nor did the adults on the field trip. Tragic.
