Why don't most US cell phones work outside the US?

What makes for instance the European phones different?

I read that Sprint and AT&T can accomodate calling/roaming overseas. One can buy or rent.
They mention GSM.

I am just back from Europe. On the way (Dulles airport) I saw a stand offering cell phones that work overseas at $0.65 per minute but I did not check out the details.

It turns out our rental car broke down and a cell phone would have come in quite nicely.

On the way back to the US, I picked up an ad from Rentacell; they will rent you a phone at ~ $39 per week, $0.69 per minute inland (this was a German company so this means within Germany) and $1.11 for other countries. These rates are base on $1 US = 0.91 Euros, so the rates are bount to be higher now.
