The first place I would start would be the American Heart Association web site. Contains patient prevention tips, keys to lifestyle modifications, and other "vital" information.

The Hypertension Network is a nice web site that clears the confusion about different medications and procedures and presents it in an understandable manner.

Although aimed at healthcare professionals , the Cardiology Compass by Dr. Murphy from the Washington University School of Medicine is a concise index site containing EKG instruction, cardiac imaging, and patient instruction.

Heartpoint contains healthy food information and other sensible ideas to keep your ticker in tip top shape. Aimed for the layperson.
