YOUR EMERGENCY ROOM VISIT: WHAT TO TAKE WITH YOU

7. YOUR LIVING WILL or ADVANCE DIRECTIVE. Not everyone has one of these, or even knows what one is. Put very simply, a living will or advance directive is a document in which you provide instructions about health care you do, or do not, want, should you be unable to make decisions at a later time. You can also designate someone to make decisions for you. For more information on living wills and advance directives, visit http://www.hcfa.gov/pubforms/advdir.htm. For information on registering your living will in a national database, accessible to hospitals around the country via telephone, visit http://www.uslivingwillregistry.com.

Ideally, you will never need to visit an Emergency Room. But if you ever do require emergency medical care, being prepared and having the above information written down will help reduce stress in an already stressful situation.
